Good news! Fields Chiropractic Clinic offers DOT physicals without having to set an appointment. That’s right, you can just walk in and get your DOT physical done without the long wait.

DOT Physical
All drivers who need a medical certificate card for their CDL license must get a DOT physical by a certified medical examiner that is listed on the National Registry of Certified Medical Examiners.  Medical certificate cards are good for a maximum of two years.  Depending on the drivers health they may be required to get recertified in less time.  Stay on top of your certification and don’t let it expire.

Dr. Fields offers same day appointments for DOT Exams.  All qualified drivers will get a medical card the same day.  Dr. Fields files all test results with the Federal Motor Carrier Safety Administration has required.

Did you Know?

Beginning May 21, 2014 all Dot Physicals must be preformed by a Certified Medical Examiner that is listed on the National Registry of Certified Medical Examiners.

Tips for a Successful DOT Physical:

  • Make sure to get copies of the long form and short form for your records.  The long form is the three page physical exam that the doctor fills out during the exam.  The short form is the medical examiners certificate card.
  •  Make sure to stay current on all waiver/exemptions and all skill performance evaluation certificates.
  •  If wearing contacts make sure to carry an extra pair of eyeglasses.
  •  If required to wear a hearing aid make sure to carry an extra battery.
